Introduction
BISAP is scored on a scale of 0 to 5, based on how many of 5 characteristics the patient has within 24 hours of presentation:
- blood urea nitrogen level >25 mg/dL
- impaired mental status
- systemic inflammatory response syndrome
- age >60
- pleural effusion on imaging studies.
A cutoff score of 3 predicts mortality. Death occurred in 18% of cases with BISAP scores >= 3 versus 1% of cases with scores < 3. A BISAP score >= 3 had a sensitivity of 71%, a specificity of 83%, a positive predictive value of 18%, and a negative predictive value of 99% for predicting mortality. A BISAP score >= 3 also predicts organ failure (odds ratio, 7.4), & pancreatic necrosis (RR = 3.8).
