Indications

* 7 prenatal nurse home visits & 2 postpartum visits decreased all-cause maternal mortality

* addition of home visits for the child up to age 2 years (mean number of visits, 33), plus developmental screening & referrals effectively eliminated preventable childhood mortality (0% vs 1.6%)[2]

Complications

* pharmacist should be included as a member of the care team[4]
