Introduction

8 mg/kg/day IV/IM

> 80 mL/min 8 mg/kg every 24 hours
60-80 mL/min 8 mg/kg every 24 hours
40-60 mL/min 8 mg/kg every 24 hours
30-40 mL/min 8 mg/kg every 48 hours
20-30 mL/min 8 mg/kg every 48 hours
10-20 mL/min 8 mg/kg every 72 hours
< 10 mL/min 8 mg/kg every 96 hours
