parathyroid adenoma

Epidemiology

• ~75% women, usually 30's

Pathology

• usually single, well circumscribed
• may deform esophagus or trachea

Microscopic pathology

• cellular, chief cells & some oxyphil cells (see parathyroid gland)
• delicate capillary network
• microfollicles common
• rarely papillary
• minimal mitotic activity
• may see bizarre nuclei
• encapsulated, no capsular invasion
• no vascular invasion
• may have hemorrhage, cholesterol clefts, fibrosis

Genetics

• t(11;11)(q13;p15) involving CCND1 with the parathyroid hormone (PTH) enhancer

Clinical manifestations

• hypercalcemia
• hyperparathyroidism
