Etiology
- antibiotic ineffective against pathogen
- undrained pus, particularly in abdomen or pelvis
- antibiotic therapy too brief
- endocarditis
- osteomyelitis
- lung abscess
- tuberculous & functional infections
- patient non-compliance
- colonized foreign body
- inappropriate dosage & delivery of antibiotic
- failure to recognize immunodeficiency syndrome
- inducible expression of resistance, especially Gm- aerobes
- incorrect diagnosis
