anisocoria

Introduction

Unequal size of the two pupils, significant is > 1 mm difference.

Etiology

• Horner's syndrome on the side of the smaller pupil
  • ptosis may be present
• 3rd nerve palsy on the side of the dilated pupil
  • occulomotor abnormalities are generally present
• benign (simple) anisocoria
• monocular pseudophakia

Clinical manifestations

• in benign anisocoria, pupil inequality is variable
  • may change from 1 hour to the next
