right middle lobe syndrome

Introduction
Recurrent atelectasis associated with isolated bronchiectasis of the right middle lobe of the lung.
Etiology
- compression of right middle lobe by lymph nodes
- acute angle of right middle lobe origin
- narrow opening of bronchus
- long bronchus
- lack of collateral ventilation
- Mycobacterium avium complex (Lady Windermere syndrome)
Clinical manifestations
- recurrent bronchiectasis & atelectasis of right middle lobe
- many patients are asymptomatic
Radiology
- diagnosis is frequently made as incidental finding on chest X-ray
- band like opacity seen on lateral film extending from hilum to anterior chest wall at diaphragm
