Etiology

• acute pyelonephritis associated with reflux nephropathy
• nephrolithiasis (xanthogranulomatous pyelonephritis)

Pathology

• xanthogranulomatous pyelonephritis

Clinical manifestations

• may present insidiously
• renal insufficiency
• hypertension
• salt wasting

Laboratory

• low urine osmolality
• hyperkalemia

Radiology

• computed tomography (CT)
• magnetic resonance imaging (MRI)
• renal ultrasound
• small kidneys
• asymmetric cortical scars

Management

• surgical resection of abscessed areas
• antibiotic therapy
