Procedure
- grade 1: asymptomatic or mild headache
- grade 2: moderate to severe headache; cranial nerve palsy
- grade 3: somnolence, confusion or minor focal neurologic signs
- grade 4: impaired consciousness; focal neurologic signs
- grade 5: coma, posturing
