Procedure

  • an emergency departement (ED) protocol that involves
    • volunteer services program to help geriatric patients
    • specifically designed geriatric ED space
    • clinical care protocols
    • early involvement of inpatient palliative care
    • dedicated staff training
  • nurse practitioners (NPs) in the ED conduct a detailed assessment of geriatric patients with a high risk for future hospital use, as indicated by an ISAR score of >= 2
