Management
- venous thromboembolism should be treated initially with unfractionated heparin until INR is therapeutic for at least 24 hours
- LMW is cleared by the kidney & has a longer 1/2 life than unfractionated heparin
- unfractionated heparin is cleared by the reticuloendothelial system
