glyburide/metformin (Glucovance)

Indications

• treatment of type-2 diabetes mellitus

Contraindications

• renal insufficiency
• avoid in patients with congestive heart failure
• caution in patient > 80 years of age
• caution in patients with hepatic disease or excessive alcohol consumption

Dosage

• start 1 tab PO QD
• up to 2 tabs PO BID
• take with meals

Tabs: glyburide (mg)/metformin (mg) 1.25/250, 2.5/500, 5/500
