right ventricular hypertrophy (RVH)

Introduction

Electrocardiographic (ECG) criteria:

• R > S in V1 & V2
• S > R in V5 & V6
• R in V1 > R in V5
• right axis > 110 degrees
• QRS < 112 msec (not right bundle branch block)

Other causes of R in V1:

• right bundle branch block - prolonged QRS
• posterior myocardial infarction
  • generally occurs in association with inferior wall MI
  • Q's in II, III, aVF from inferior wall MI
• Wolf Parkinson White (WPW) type A (accessory tract anterior to mitral valve)
